Summer Programs

Medical Disability Information
If your child has a learning, physical, or medical disability, you must contact summer programs staff at 410-735-6215 or ctydisabilities@jhu.edu to discuss those needs in the context of the academic, social, and residential aspects of the CTY program. Requests for accommodations must be submitted to our office by May 15. You must also indicate any disabilities in the Medical Information Page.


Requests for Accommodations and Support
The forms below will allow you to notify us of any learning, physical, or medical disability that your child may have. Please note that any requests for accommodations must be accompanied by current documentation from a qualified individual knowledgeable about the student's disability. For more information about required documentation please refer to the "Documentations and Accommodations" section of the site information packet.

Forms to request accommodations and support are available in either PDF or MS Word format. Forms available in PDF format must be printed and completed by hand. MS Word forms will allow you to enter the information directly on the form. Please note that most forms require an original signature and even if you use the keyboard method to fill them out, you must print the form and provide your hand signature where indicated. All forms that require an original signature may be faxed to 410-735-6197 or mailed to:

The "Request for Accommodations Form" does not require an original signature and may be emailed to ctydisabilities@jhu.edu. If you have the facility of a document scanner, you may scan any forms that require a signature and e-mail them to this address.

Student Medical Forms
After assignment to a course, students are required to submit a set of medical forms, some of which require a physician's signature. Site specific medical forms are made available as part of the site information packets. You may also obtain copies of these forms below. Please note that forms found under each site are required at that location.

Medical forms must be submitted to the CTY Registration Office no later than May 15. Please keep a hard copy of these forms for your records and bring or send a duplicate copy to the site. Completed forms may be sent to:

CTY’s Medical/Disability Committee will review submitted documentation and your request for accommodations and/or support and, in coordination with Johns Hopkins University’s Director of ADA Compliance and Disability Services, will determine accommodations and/or support that CTY will provide for your child while attending CTY Summer Programs.
