You have two options for completing your application. You may fill it in using your keyboard and then print it (preferred method), or you may print it first and complete it by hand. Whichever method you choose, please mail the completed application and required supporting documents to:

Candidates may not be employed until we have at least two references on file: one from an employer who has supervised your work experience with children and one from your most recent employer in work relevant to CTY.

There are two versions of the Employment Reference Form. The first is a .pdf documented to be downloaded and completed by hand. Download the .pdf document below, print it out, fill out the box at the top of the first page with your name and the position(s) for which you are applying, and then forward it to the referrer. The second version is an MS Word form to be completed by keyboard. Download the Word document below, type your name and the position(s) for which you are applying into the fields in the box at the top of the first page, and then email this electronic document to the referrer for completion.
