Nomination Form and Guidelines

Who is Eligible to Nominate?
CTY students have a one-time opportunity to nominate teachers as an SDB fellow. If you have previously nominated a teacher, please allow other students to exercise this privilege (special note).


Guidelines for Students

The Nomination Form - Please complete the nomination form and write an essay. Then print two copies of the form and essay, mailing one set to the address below and keeping the other set for your records.

The Teacher Nomination - Please consult with the teacher you wish to nominate. Confirm you have spoken with the nominee by checking the box at the bottom of the form. The person nominated can be your current or former teacher.

The Essay - Write an essay of no more than 500 words describing the teacher’s contribution to your experience as a highly able student.

  1. State the grade level and subject in which the teacher contributed to your growth as a highly able student.
  2. Be sure to include classroom examples of this contribution.
  3. Check your spelling, punctuation, and grammar.
  4. At this time, please do not share your essay with the teacher. A copy of your essay will accompany the teacher’s certificate of recognition.

No later than October 14, 2011, mail the completed form and essay to:
